DVA Community Nursing Services

Border Community Nurses is an approved DVA Community Nursing Provider. This means that we are able to provide nursing services to DVA clients and the cost for service is funded.

Who can access Community Nursing services through DVA?

To receive Communiuty Nursing Services for an assessed clinical need, a person must have either a Veteran Gold Card or a Veteran White Card.

A client who has a Veteran Gold Card will have their nursing service funded.

A client who has a Veteran White Card will have their nursing service funded if the care they need is due to an accepted service-related injury or condition.

I am a Veteran Card Holder and I require nursing assistance; how do I get started?

You will need a referral for Community Nursing Services (D904) if you are new to receiving Community Nursing Services, are starting a new episode of care or if your referral is older than 12 months old. Who can refer me to Border Community Nurses for approved nursing services?

To provide nursing services to a client, a referral must come from an authorised source, which can be a:

  • General Practitioner (GP)
  • Specialist
  • Nurse Practitioner specialising in a community nursing field
  • Treating doctor in hospital
  • Hospital discharge planner

Clinical Home Care

High Intensity Clinical Care

Border Community Nurses provide clinical nursing care in your own home. As trained Registered Nurses, we can offer high intensity clinical care support for medical needs such as:

  • Complex Bowel Care
  • Enteral Feeding and Management (PEG)
  • Diabetes Management and Care
  • Pressure Care and Wound Management
  • Stoma Care
  • Urinary Catheter Care
  • Medication administration
  • Prescribed injections

And other services that require clinical skills.

Personal Care

Many seniors worry about losing their dignity and their independence as they age and become less able to look after themselves. When you can no longer accomplish personal hygiene tasks such as bathing and toileting without help, it is often better to have a non-family member assist. This assistance preserves the dignity of both family members and your own.

Border Community Nurses can assist you with activities of daily living such as:

  • Assisting with personal activities
  • Toileting and continence assistance
  • Showering and bathing
  • Grooming and personal hygiene
  • Helping to get in and out of bed
  • Dressing and undressing
  • Medication assistance and reminders
  • Assist life stage transition and support
  • Community Nursing Care for High Needs

And many other daily tasks that may require a little extra assistance.

Brokerage Services and self-managed Home Care Packages

Border Community Nurses provide brokerage services to aged care and disability support providers. We maintain our honest and professional approach with our brokerage partners and keep services affordable by offering consistent pricing. This facilitates case managers in accurate budgeting for individual clients.

Palliative Care

Palliative Care at home allows individuals to end their life’s journey in comfortable, familiar surroundings. Through our Palliative Care Services, we will work with you, your loved ones and your GP and local palliative care teams to plan your end-of-life journey and provide you and your family with the assistance, support and holistic care needed to achieve your goals.

Can You Have Palliative Care at Home?

Absolutely. Border Community nurses are here to help support you or your loved one in the comfort and familiarity of home.

Does Palliative Care Mean Death?

Palliative Care doesn’t necessarily mean death. Palliative Care is only recommended when a patient is diagnosed with a life-threatening illness, but death isn’t a guarantee. The goal of Palliative Care is to provide support and give the patient the highest quality of life possible as they battle their illness, and in the case of death, to make that transition as painless and dignified as possible.

Saturday$130.00 Per Hour
Sunday$140.00 Per Hour
Public Holidays$170.00 Per Hour
  • Costs for wound dressings supplied by Border Community Nurses may attract additional charges.
  • Travel charges may apply to clients residing more than 10km outside the Albury / Wodonga area.
  • Travel charges are charged at $1.40 per km
  • Service for National Disability Insurance Scheme participants will be charged at the current Pricing Arrangements - for delivery of health supports by a Registered Nurse.
